Bone mineral density of the proximal humerus in patients with glenohumeral osteoarthritis : intraoperative assessment and impact on treatment alternatives

abstract

The bone quality (bone mineral density - BMD) of the proximal humerus might be crucial for clinical outcome of some new and evolving shoulder arthroplasty systems like the stemless humeral components, which relies on uncemented pressfit fixation in the spongious bone of the humeral metaphysis. Systemic measurement of BMD density for diagnosis and staging of osteoporosis is not a practicable option for shoulder surgery.

The purpose of our study to use new DEXA measurement tool (MicroCT) as reference value for evaluation of clinically available options for BMD assessment (DTI, haptic assessment, PqCT), as well as in relation to mechanical properties (indentation measurements with MicroMotion). If a threshold value of BMD with good specificity and sensitivity (or strong predictive power) could be determined, then an intraoperative tool for BMD measurement would potentially provide information for intraoperative decision making.

The primary endpoint of the study will be the relation of bone mineral density measured and assessed using different tools.
