Erectile Function Preservation after Therapy of Localized Prostate Cancer: I125 Prostate Seed Implant Brachytherapy Versus Radical Prostatectomy

Objective: The aim of this study was to determine the erectile function (EF) preservation after treatment of localized prostate cancer with I125 Prostate Seed Implant Brachytherapy (BT) with 145Gy versus Radical Retropubic Prostatectomy (RRP)
Materials and Methods: All patients receiving either RRP or BT for localized prostate cancer at the Kantonsspital St Gallen between 2005 and 2008 were asked to participate in this study. The survey was filled out pretreatment and 6 weeks, 6, 12, 24 and 36 months after therapy. Normal baseline EF was defined as a sum of >7 for questions 3 and 4 of the IIEF.
Results: 286 pts of the 301 consecutive pts agreed to participate in the survey. Normal preoperative EF was found in 189 pts (66%). Treatment consisted of RRP in 112/189 cases (59%), BT was performed in 77/189 cases (41%). In 33% of the operations uni- or bilateral nerve sparing RRP was performed. EF after BT vs. RRP was normal in 56% vs 9.7% of the patients at 6 weeks, 58% vs 8% at 6 months, 51% vs 6.4% at 12 months, 50% vs 11.6% at 24 months and 61% vs 19% at 36 months (p<0.01).
Conclusion: Our data demonstrate a significantly better erectile function preservation in patients receiving brachytherapy for localized prostate cancer in comparison to patients receiving radical retropubic prostatectomy.
