Ein 61-j Frührentner wird bewusstlos aus einer brennenden
Wohnung geborgen. Befunde in der Zentralen Notfallaufnahme: GCS 14,
kreislaufstabil, agitiert. Die aBGA zeigte einen FCOHb von 36.2%
sowie eine metabolische Azidose mit einem Laktat von 7.2 mmol/l. Bei
Verdacht auf eine Mischintoxikation mit Kohlenmonooxid und Zyanid,
Gabe von insges. 5 g Cyanokit (Hydroxocobalamin), Intubation.
Problemlose Extubation am Folgeta und nach insges. 9 Tagen
Entlassung nach Hause. Die Analyse des Zyanidspiegels ergab einen
Wert von 0.5 mg/l, der Alkoholspiegel im Blut betrug 2.2 Promille.
